Applications can be picked up and completed applications can be dropped off at the following locations:
City of Goodwell, 104 S Main, Goodwell, Oklahoma, Guymon Chamber of Commerce, 206 NW 5th, Suite 3, Guymon, Oklahoma, City of Hooker, 111 S Broadway St, Hooker, Oklahoma
Disbursements may be up to $2,500 while funding is available.
Read the instructions carefully and complete all of the information.
Complete both post office and physical address even if they are the same.
You must attached documents for one of the three options. You can include pictures, the cost of replacing household and personal items.
